Targeted Case Manager
Full-TimeProvide Targeted Case Management services to eligible individuals, including assessment, care planning, and community resource coordination. Kentucky licensure or eligibility required.
Behavioral Health Case Manager
Full-Time / Part-TimeSupport individuals with behavioral health needs in accessing services and achieving their personal goals. Experience in behavioral health or social services required.
Care Coordinator
Full-TimeCoordinate services and resources for clients, working closely with healthcare providers, community organizations, and families.
