As an authorized Kentucky Behavioral Health Services Organization (BHSO), Wings of Freedom Care LLC is ready to serve eligible clients across five licensed service areas. All services are subject to eligibility verification and Medicaid FFS coverage confirmation.
Our Five Licensed Services
Assessment
Evaluating clients' behavioral health needs and identifying the most appropriate services and supports to address those needs.
Crisis Intervention
Providing timely, compassionate support to individuals experiencing behavioral health crises, helping stabilize and connect them to ongoing care.
Day Treatment
Providing structured behavioral health treatment and support programs designed to promote recovery, skill-building, and community integration.
Service Planning
Developing individualized plans that address each client's unique behavioral health needs, goals, and strengths in collaboration with their care team.
Targeted Case Management (TCM)
Helping eligible clients access healthcare, mental health treatment, housing resources, transportation, and community services through dedicated care coordination.
About Targeted Case Management
Targeted Case Management (TCM) is a service that helps eligible individuals access and coordinate the behavioral health, healthcare, and community support services they need. A dedicated care coordinator works with you to develop a personalized plan and connects you with providers and resources in your community.
How TCM Works
Initial Contact
Reach out to our team by phone, form, or referral. We will schedule an initial conversation to learn about your needs.
Eligibility Screening
We verify your eligibility for Targeted Case Management services, including Medicaid FFS coverage and service criteria.
Assessment
Your care coordinator conducts a comprehensive assessment to understand your strengths, needs, and goals.
Care Planning
Together, we develop an individualized service plan that outlines your goals and the steps to achieve them.
Ongoing Coordination
Your coordinator actively connects you with services, monitors your progress, and adjusts your plan as your needs evolve.
All services are available to eligible Kentucky Medicaid Fee-for-Service (FFS) members, subject to eligibility verification, coverage confirmation, and provider billing approval. Wings of Freedom Care LLC is licensed as a Behavioral Health Services Organization (BHSO) by the State of Kentucky. Private-pay arrangements may be available upon inquiry.
